Men's Empowerment Initiative

Milwaukee Health Services, Inc. invites Black men to complete a two-minute survey through Empowerment MKE, our men’s health initiative focused on awareness and early detection of colon, prostate, and lung cancer. Our goal is to collect 1,000 completed surveys to better understand men’s perspectives on cancer screening and inform our community outreach efforts. Participation is open to MHSI patients and members of the public. This is a survey, not a cancer screening or medical test.

How to Take the Survey

Click the Men’s Cancer Screening Survey link or scan the QR code on the survey flyer. The survey takes approximately two minutes to complete and is available in English. Please complete the survey only once.
