Appointments may be made for routine visits or new problems that you may be experiencing. If you wish to schedule a time please call the provider’s office for a list of available appointments to meet your needs. You can also fill out the form below to have someone reach out to you. 

Patient Rights & Responsibilities

Milwaukee Health Services, Inc. (MHSI) respects each patient and is dedicated to giving the best care possible. Patient Rights and Responsibilities explain what you can expect from us, and in turn what we expect of you.

As a patient, you have the right to:

  • Take part in your health and treatment
  • Know the names of the people caring for you
  • Be treated with respect and dignity in a safe private setting
  • Be informed about your illness and treatment, including options for your care
  • Interpreter services and/or special devices if you are limited-English speaking, deaf, or hard of hearing and/or visually impaired
  • Refuse to be examined, observed, or treated by students without jeopardizing your access to care
  • Respect for your cultural, social, spiritual, personal values and beliefs
  • Express concerns about your care or safety and receive a quick reply
  • Know about services available through MHSI
  • Privacy of your health records
  • Change medical providers at MHSI

As a patient, you have the responsibility to:

  • Tell your medical provider about your illness or problems
  • Ask questions about your illness or care
  • Show respect to both care givers and other patients
  • Arrive 15 minutes prior to your scheduled appointment time to register
  • Cancel or reschedule appointments so that another patient may have that time slot
  • Use medications or medical devices for yourself only
  • Give written permission to release your medical records to MHSI
  • Call your pharmacy for refills at least 3 days prior to running out
  • Bring in all your medication bottles or an up to date list to every visit
  • Speak up if you have a concern about your safety as a patient
  • Provide accurate information about your insurance or lack of insurance
  • Pay your sliding fee at the time of service
  • Pay your insurance co-pay at time of service and all other charges by the due date

  • Medicare
  • Commercial Insurance
